Description

Zep® tablets are produced by Terrace Pharmaceuticals and marketed by Mefro Pharmaceuticals. Zep® is known as an anticonvulsant or anti-epileptic drug used to decrease nerve impulses that cause seizures and pain and to relieve certain types of nerve pain (such as trigeminal neuralgia). The medicine also manages manic-depressive disorder and alcohol withdrawal symptoms. The active substance contained in the drug is Carbamazepine, an anticonvulsant similar in its structure to tricyclic antidepressants. Carbamazepine works by reducing the spread of seizure activity in the brain and restoring the normal balance of nerve activity. Zep® also has shown its positive effect on managing cranial diabetes insipidus, as its active ingredient Carbamazepine has the ability to decreases the urinary output and the free water clearance and increase the urinary osmolarity.

Indications:

- control epilepsy, a condition associated with repeated generalized tonic-clonic and partial seizures (fits) (should not be confused with the absence or myoclonic seizures);

- treat trigeminal neuralgia, where there are sudden, repeated attacks of facial pain;

- control mania, a mental condition with episodes of overactivity, elation or irritability;

- prevent bipolar mood disorder where periods of mania alternate with periods of depression;

- treat alcohol withdrawal;

- manage the urinary volume in patients with cranial diabetes insipidus (should not be confused with diabetes mellitus or sugar diabetes) due to its antidiuretic activity.

MOA (mechanism of action):

Carbamazepine is a sodium channel blocker. It works by binding to voltage-gated sodium channels in their inactive conformation, which prevents repetitive and sustained firing of an action potential. The active ingredient Carbamazepine stabilizes hyperexcited nerve membranes, inhibits repetitive neuronal discharge and reduces synaptic propagation of electrical impulses. Carbamazepine also possesses anticholinergic, central antidiuretic, antiarrhythmic, muscle relaxant, antidepressant, sedative, and neuromuscular-blocking properties.

Dosage

Adult dose for Epilepsy:

- Initial dose: 200 mg orally 2 times a day.

- Maintenance dose: 800 to 1200 mg per day.

- Maximum dose: 1200 mg per day.

Children dose for Epilepsy:

- 5-10 years: 400-600 mg a day.

- 10-15 years: 600-1000 mg a day.

In elderly patients, the dose should be prescribed by a doctor for each individually.

Adult dose for Trigeminal Neuralgia:

- Initial dose: 100 mg orally 2 times a day.

- Maintenance dose: 400 to 800 mg per day.

- Maximum dose: 1200 mg per day.

Elderly dose for Trigeminal Neuralgia:

- Initial dose: 100 mg 2 times a day.

- Maximum dose: 1200 mg per day.

In elderly patients, the dose should be prescribed by a doctor for each individually.

Adult dose for Manic-depressive disorder:

- Initial dose: 400 mg per day.

- Maintenance dose: 400 to 600 mg per day.

- Maximum dose: 1600 mg per day.

Elderly dose for Manic-depressive disorder should be prescribed by a doctor for each individually.

Missing a dose

If you miss a dose of Zep® you should take it as soon as possible. If it is the time for the next dose, should continue your regular dosing schedule. Do not take a double dose.

Overdose

If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately. Do not exceed the doctor’s recommended dose.

Storage

Store at a room temperature below 25°C (77°F) away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the sight and reach of children.

Contraindications

Do not take Zep® if you are allergic to Carbamazepine or any other components of this medicine.

Avoid eating grapefruit or drinking grapefruit juice while using this medication as it can affect the efficacy of medication and increase the chance of side effects occurrence.

Do not take Zep® tablets if you have a history of bone marrow suppression.

Avoid taking MAO inhibitors (like isocarboxazid, linezolid, moclobemide, phenelzine, rasagiline, safinamide, selegiline etc.) during treatment with this medication.

Do not take the medication if you have a heart disease, high blood pressure, high cholesterol or triglycerides.

Do not take Zep® if you have liver or kidney disease.

Patients of Han Chinese or Thai origin should be blood tested before starting the course of treatment with Zep® tablets.

Do not drive, use machinery, or do any activity that requires alertness, as Carbamazepine may cause dizziness, drowsiness, blurred or double vision.

Do not give Zep® to patients under 5 years of age.
